LIABILITY AND RELEASE

Participant Waiver & Release

I understand that participation in Campanella Summer Music & Fairytale Camp includes singing, movement activities, games, arts and crafts, and other supervised educational activities. I certify that my child is physically able to participate in these activities.

In consideration of my child's participation, I release and hold harmless Campanella Children's Choir & Educational Center, its directors, employees, volunteers, and Glenview Community Church from liability for injuries, losses, or damages arising from participation in camp activities, except as may result from gross negligence or willful misconduct.

In the event of an emergency, I authorize Campanella staff to obtain appropriate medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred.

I grant permission for Campanella Children's Choir & Educational Center to photograph and/or record my child during camp activities and to use such photographs, video, or audio recordings in Campanella publications, social media, promotional materials, website content, and other educational or marketing purposes without compensation.

I certify that the information provided in this registration form is accurate and complete.

Early Bird Rate: $145 if you register by July 20th ($160 after)

Sibling Discount: $20 off each additional child
